We continue to request that families complete the Google form for reporting on vaccination status and interest in our in-school testing programs. The form is linked here: NMRSD Student Consent Form. If you have not completed the form, please take the time to do so. It only takes a few minutes.
When you complete the google form there is a link to consent with CIC Health. Please make sure that you follow that link and complete your consent with CIC Health for testing.
Some of you have completed the Google form and indicated that you want your child to participate in testing, but you are not showing up in the CIC health database as having provided consent. CIC Health is the program that we are using to manage the testing and legal consent . If you completed the google form, but did not sign consent using the CIC health link that was on the form, please go here and complete the consent form. It takes about 24 hours for your consent to be uploaded into our database.
Please note: No student will be allowed to participate in the Test & Stay program going forward unless consent is completed in the CIC health database. This is a requirement of DESE and DPH.
If your child is identified as a close contact at school you will have to pick your child up and he/ she will have to quarantine at home for 7 days, with return on day 8 if an outside test is negative on day 5, OR quarantine for a full 10 days if no test is done.
If your child becomes sick at school, you will have to pick your child up and have an outside test done showing a negative result before he/she can return to school.
In order for our test and stay program to be successful, not only do we need consent for testing, but we also require timely notification if your student is sick and tests positive for COVID-19.
Please inform the school nurse as soon as you know of a positive case in your family so that contact tracing can begin immediately and others can be notified as soon as possible. Contract tracing begins 2 days either before onset of symptoms or 2 days before a positive test result if there are no symptoms.
